[Nordskogen] WW - notarized minor waivers

Greetings!
If you are bringing a minor that you do not have legal custody for to WW
(I.e., you are not the child's biological, custodial, or legal parent or
guardian) you will need to bring a notarized medical waiver that has been
signed by the custodial parent or legal guardian. The form can be found
here: http://www.sca.org/docs/treatminor-notary.pdf
I will be at the Nordskogen meeting tonight with my notary stamp and some
of the forms. I must witness the parent or guardian's signature - I cannot
notarize a blank form.
It is very important that you bring this form with you. Without it, we
cannot admit the child to the event. Last year, we were able to resolve
these issues via having forms faxed - we do not have that resource this
year.
I am also available for notary duty any evening this week after 6pm, if
you can make it to my place in Roseville, or during the day if you are in
downtown Minneapolis anywhere on the skyway system.
